Employees: Attest to Your COVID-19 Vaccination Status

Dear DePaul employees,

As previously communicated by the Office of the President, employees are required to be vaccinated for COVID-19. To make the process as simple as possible, employees will be required to attest to their COVID-19 vaccine status through a secure third-party platform.

Attestation instructions
Employees should only submit their vaccination information after they receive their second dose of the Pfizer or Moderna vaccine, or following the single dose of the Johnson & Johnson vaccine. Furthermore, employees who are also DePaul students and recently uploaded their immunization records through Campus Connect, will also need to complete this process to ensure compliance. Student employees only need to submit through Campus Connect.

COVID-19 vaccine and attestation deadlines
  • Aug. 23: College of Law employees must receive at least one dose of the COVID-19 vaccine.
  • Sept. 3: All employees, minus College of Law, are required to receive all recommended doses of the COVID-19 vaccine.
  • Sept. 10: All employees, except the College of Law, are required to complete their vaccination attestation through the secure third-party platform.
  • Sept. 20: College of Law employees are required to complete their vaccination attestation through the secure third-party platform.
