COVID-19 vaccination requirements

In compliance with Gov. Pritzker's executive order and in the spirit of caring for each other and for our surrounding community, DePaul is requiring vaccinations for COVID-19. This requirement applies to students, faculty, staff, vendors and others who provide services to campus.

While the deadlines for current students, faculty and staff (full and part time) to submit their information have passed, you may still upload your information at any time if you have not yet done so. Students who will begin taking face-to-face classes and new faculty and staff also should follow the directions below.

Employee vaccination records will be maintained separately from employee personnel files, will be stored securely in accordance with the requirements of DePaul’s Access to and Responsible Use of Data policy, and will only be shared only to the extent necessary to administer the program and ensure compliance with the university’s program and Governor Pritzker’s Executive Order.

Vendors & volunteers

Vendors and others who provide services to DePaul must be vaccinated for COVID-19 or meet other requirements. DePaul departments and areas are responsible for collecting the compliance forms. Learn more and download the forms on the Vendor & Volunteer Vaccine Compliance webpage.

How to schedule a vaccine appointment

There are multiple ways to receive a COVID-19 vaccine; some require appointments and some don't. Please check the information associated with the vaccination site closely to see if an appointment is required.

Have questions about how to get the vaccine? Send an email to DePaul staff can answer questions about vaccine eligibility and appointment locations. Please note that staff will not book appointments for you.

Chicago and Illinois residents

Faculty, staff and students ages 12+ can seek appointments through the following methods:

    • Through city-run mass vaccination sites. Learn more about the sites here. All city of Chicago-operated sites accept walk-ins. Appointments are preferred but not required. Appointments can be made via the centralized vaccine booking site Zocdoc. Please visit, confirm your location and eligibility and receive real-time appointment availability.

    • Through pharmacies. Each has its own registration system which can be found at the following links:
        • Walgreens or call 1-800-WALGREENS (1-800-925-4733)
        • Mariano's
        • Walmart or call 833-886-0023; Option 1
        • Jewel Osco or call 877-723-3929
        • CVS
        • Chicago Costco Pharmacies or call (773)-360-2052 for Lincoln Park location (2745 N. Clybourn Ave.) or (312) 492-3601 for Chicago IL Medical District location (1430 S. Ashland Ave.)

    • Through your health care provider. This includes your primary care provider, health clinic or hospital where you received medical care in the past. Please reach out directly to your providers to schedule appointments.
