Registration Guide
Program Information - Program name, date, location & description of activities. For students, include name and email of Faculty Advisor
External Entity - Information of external entities that may be involved, including contractual agreement if applicable
Variance - Description and documents for Programs or Events eligible for a variance from the Protection of Minor Children policy
Authorized Adults - The employee name and DePaul email for all adults involved in the Program/Event
Attendees - Name and birth date/age of participating minor children under the age of 18 and parent/guardian signed waiver that releases DePaul University
