- Contact Information: name, email and phone
- Program name, date and time
- Company address
- Date and location of program
- Description of activities involved in the program
- DePaul location (address/building)
Adult Supervision
- Name and email of adults involved in the Program/Event (At least one adult is required to submit registration.)
Attendees
- For minor children under the age of 18:
- Name and birth date of all participating children
- Document image of a parent/guardian signed waiver that releases DePaul University
