Parking Event Request Form

Please use this form to request event parking or discount vouchers. These requests should be submitted a week in advance so we can confirm availability and accessibility. A certificate of insurance (COI) is required for any special event that reserves the entire parking lot. It must include the following under the description: "DePaul University, its officers, directors, trustees, employees, and agents shall be included as an additional insured as their interest may appear on the general liability policy where required by written consent."
