Students: Complete the COVID-19 Vaccination Survey

DePaul wants your input.

Earlier this year, the Chicago Department of Public Health asked DePaul's COVID-19 Response Task Force to survey the DePaul community to help inform vaccine distribution efforts. Please complete the quick COVID-19 Vaccination Survey by Sunday, April 18. Your feedback will help DePaul understand students' vaccination plans, help quantify needed vaccines and provide insight into what resources students need.

Participation is voluntary. All responses will remain anonymous and confidential, and will only be used to help DePaul with its vaccine plans and advocacy efforts.

Student employees who completed a similar survey in early March should complete this survey as well to ensure their responses are represented within the student population.
