Collaboration Agreement

To support our service-learning students, partnerships require a standard master Collaboration Agreement between DePaul and our community organizations. Please be prepared to submit your collaboration agreement with the intake form.

You can find our standard Collaboration Agreement here. Once signed by both parties, the agreement is valid for up to 5 years.

Note: You will not be able to submit the intake form without a signed community partner Collaboration Agreement.
